Matrix House Call Program Available for MA Members

Highmark Blue Cross Blue Shield Delaware is working with Matrix Medical Network to offer a yearly in-home health risk assessment to eligible members of our Medicare Advantage (MA) plans, as well as some other benefit plans.

House Call – What It Is

Qualifying patients will be offered a member wellness program, the Matrix House Call. During the house call, a nurse practitioner will complete a health risk assessment, review medications, and answer questions.

In addition, the nurse practitioner will provide valuable insights into the patient’s overall health, including home safety, domestic abuse, behavioral health, and dietary nutritional needs that may be difficult to detect in a clinical setting. If someone is clinically unstable, the nurse practitioner will call you as the member’s primary care physician (PCP) that day.

House Call – What It Isn’t

House Call visits do not replace regular doctor visits or annual wellness exams. Matrix clinicians do not prescribe medications or perform and order invasive tests or procedures. Matrix encourages members to speak with their PCP about follow-up and further healthcare decisions.

The House Call visit is scheduled through the health plan and the results are mailed to you and your patients. They will be encouraged to follow-up with you, their primary care physician. A telehealth option is also available.
